The Director of Clinical Services is a health care professional who is approved by the Board of Directors to organize and direct the Agency’s service programs.  He/she functions under the direction of the Vice President of Clinical Operations and the Governing Body and reports directly to the Vice President of Clinical Operations.

The Registered Nurse, under the direction of the Director of Nursing, is responsible for promoting the delivery of coordinated, comprehensive care to patients through the activities of assessment, planning, and provision of direct nursing care within the policies and standards of the Agency.

  • Ypsilanti/Belleville- Contingent

The Home Health Aide is a non-professional employee, trained to provide personal care and related services in the home.  She/he functions under the direction, instruction and supervision of the Field Nurse and/or the Director of Nursing.

Position available:

  • Brighton- Contingent
  • Livonia- Contingent

The Speech Therapist directs, supervises, evaluates and provides speech therapy services to Agency patients as prescribed by the attending physician and for providing Agency staff, and the patient’s physician with speech therapy consultation, as needed.

Position available:

  • Monroe – Contingent
  • St. Clair Shores – Contingent

The Medical Social Worker provides medical social services to patients in their place of residence in accordance with the Plan of Care.  The Medical Social Worker assists the physician and other team members in understanding the significant social and emotional factors to the health problems.

Position available:

  • Monroe- Contingent

The Physical Therapist is a qualified professional therapist who directs, supervises, evaluates and provides Physical Therapy Services to patients in the home as ordered by the attending physician.

Position Available In:

  • Marine City, St. Clair, Algonac, and New Baltimore Areas- Contingent

The Occupational Therapist is responsible for providing in home Occupational Therapy services to agency patients, as prescribed by the attending physician.

A Registered Nurse under general direction from the Director of Nursing, Director of Clinical Services, provides nursing care of patients; plans and oversees departmental staff nursing activities, ensuring customer satisfaction and adherence to policies, procedures, standards and regulations.

The Home Care Scheduling Care Coordinator is part of the Care Coordination team and is a position with a number of important responsibilities for the on-going daily operation of the Agency. The Care Coordinator is the primary office interface to the Agency’s referral sources and is responsible for receiving the referrals. The Scheduling Care Coordinator is responsible for day-to-day management of scheduling and assigning Agency’s patients to the appropriate professional clinicians to maximize patient care and efficient utilization of clinical staff. Care Coordinators work together to ensure optimal patient care and timely service by our clinical staff.
